This intake form is a great way to make first contact with me.  I will only be responding to forms that look to be filled out by a human being.  Forms with too little/too generic info will be discarded.

Compatibility is a priority for me, and I screen my clients not only for my safety but also seeking a sense of affinity between us.  Whenever possible, I also like to set up a time to chat on the phone.  I find phone conversations very helpful to give us both a sense for how we might connect.
